The song is a double allusion to both running from your problems for so long that they build up and you need to find some way to answer them against your will and to suicide. Either interpretation is perfectly valid.

The lyrics in the first verse detail the baggage he's created by not solving his problems in love and life. The dead bodies in his car with a massive trunk are referring to the things he's stuffed away and his from other people internally. Many songs on the album indicate depression from falling in love previously that didn't work out, such as American Wedding or Strawberry Swing, and I believe he means to say even mental health professionals and his friends can't help him, just like the cops who pull him over for the blood pooling from his trunk can't find the bodies. He's hidden them so well, for so long, no one but him even know.

The bridge at the end begging the people around him to not help him is a lot like being depressed and pushing people away even in your darkest moments, because being painfully vulnerable is extremely hard and a lot of the times friends and family can't help you. Frank comes across as very seriously willing to go through with his plan of driving into the ocean whether is good or bad. He doesn't care about the repercussions, just what it will change.
