There was a belief at the time, perhaps not unfounded, that Thatcher's government didn't care about people taking heroin because it kept them quiet so they were happy to have cheap smack on the streets. @blackaliss I always assumed Malcolm referred to Malcolm Owen of The Ruts who died of a heroin OD a year prior to the release of this song.
