I do not know the real meaning of this song but here's my own interpretation of it:

I think it's about having a SO who's suicidal and being aware of it I can't imagine anyone not feeling/having felt something like suicidal thoughts would know how it feels, and I believe the writer is aware of this when writing these lyrics. The writer goes through a list of possible things his SO might be experiencing

If you must wait, Wait for them here in my arms as I shake

"wait" being a state where the SO is trying to fight away dark thoughts the writer wants to be near them even when knowing the fact of their SO's state makes his arms shake from fear of loss and sadness.

If you must weep, Do it right here in my bed as I sleep

I believe the writer knows that he cant always give a helping hand in a situation like this, but he would still like to be close to them and perhaps offer some degree of support just by being physically near.

If you must mourn, my love Mourn with the moon and the stars up above If you must mourn, Don't do it alone

Again, I think the writer knows he may not be able to help in the mourning the thought of leaving forever. but they urge their SO not to do it alone.

If you must leave, Leave as though fire burns under your feet

i'm not entirely sure what is meant here but to me, as horrible as it sounds, the writer is asking his SO not to suffer for long.

If you must speak, Speak every word as though it were unique

The writer is asking his SO that, if they want to talk about anything, they should say everything and not leave a word unsaid. After all, this might just be their last words.

If you must die, sweetheart Die knowing your life was my life's best part

The writer feels how useless he might actually be when this is a battle one fights in their own mind, but he still wants his SO to know that they are the best part of his life even if they succumb to their thoughts in the end.

If you must die, Remember your life

The writer still feels compelled to offer some guidance by asking their SO to remember the good parts of their life

The writer conveys frustration from not being able to do anything to help.

If you must fight, Fight with yourself and your thoughts in the night

The writer here asks that his SO not fight him and push him away, but rather fight battle with their thoughts.

If you must work, Work to leave some part of you on this earth

The writer tells his SO that if they want to do anything in life before they leave, that they do something and create memories he can remember them by.

Just live Just live Just live

The writer, reaching the option of his SO fighting through and living, he is unable to continue to stand by and he expresses his desire for his SO to "just live" in a way that's similar to begging.

I apologize for this very gloomy interpretation, but I've known people with suicidal thoughts. Although they were my friends and not my SO, I have felt the futility of my attempts to help them.
