i think this whole song is from the pov of an elderly person in a nursing home. They are old, and lonely, far from their family, they feel gross, everyone around them is gross, and they are acting out, maybe even suicidal because they dont see a point in life if its just sitting in a home somewhere.
They think their family would rather shoot or cut themselves than be around them, and being in therapy together is the only thing thats brought them together.
On a darker vein of thought, its almost like the family who put them in the home is genuinely sick and tired of them, and maybe actually tries to "put them out of their misery" so to say, because of the constant "dont shoot" and "youre hurting me" lines and the whole part at the end.
Either way, its a lonely elderly person who feels forgotten about by their family and just wants the attention of their loved ones again and will do anything to get it, even hurt themself.
