I've heard stories of when my grandpa was sick, on his deathbed, and my grandma would sing this to him.

They were really really in love. One time when she had fallen and started to go down hill health-wise, my mom played this CD on a little boombox with this song, and my grandma started to sing along. And then she started to cry.

Its pretty obvious this is about one person loving another and then dying.

Well. Now she's in heaven too. And I have no doubt they are strolling hand in hand.
