I always thought that the lines "I can see her rising up out of the back seat now" and "I think he's right behind me now and he's gaining ground" were references to death catching up with him as after each he sings "But it was long ago and it was far away, oh God it seems so very far" and "objects in the rear view mirror may appear closer than they are" as in death is closer than it used to be and noticing its catching up with him
@glynf I think they are metaphors for the memories and pain associated with them hitting him. He still feels the pain as if it happened recently. Which explains the line object in the rear view mirror may appear closer than they really are.
