I don't really have time to analyse this song, but I just want to point out that the line:

Dead I am the sky, watching angels cry While they slowly turn, conquering the worm

is almost certainly a reference to Edgar Allen Poe's poem The Conqueror worm. These 4 lines from the poem sum up the connection well.

An angel throng, bewinged, bedight In veils, and drowned in tears,
and That the play is the tragedy, “Man,”
And its hero, the Conqueror Worm. The poem is about the horror of inevitable mortality, where the worm of death always wins. The inversion of this meaning in the song might have something to do with conquering death, or might just be going along with the theme of inversion shown later in the song, such as here: Dead I am the rat, feast upon the cat I don't know what this means, but it's probably important.

"Slam" is a street term for Injection (hard) drug use, so the song is most likely about using dangerous recreational IV drugs, but not dying (cheating death) in the process (conquering the worm).

"digging through the ditches" could then refer to the desperate and depraved behavior of a junky searching the lowest points of society for his next fix
