You could read it as the protagonist finally confronting his final victim, himself, whom he hates so very much for all those bad things he feels that he brought upon his family. Through things such as chemical dependencies and mental health issues he feels he has done things to the people he loves that to him seem just as bad as actually killing them.
In the end he sees no recourse but to take his own life. He spends his last breath on earth hoping against hope, that through this act he may have redeemed himself for what he put his family through and that maybe he would go to heaven after all.
It works really well as a song about suicide.
