I believe that the song is about someone who needs help or wants help from someone or something. They are fighting their demons (inner demons) and needs saving that they may or may not be getting.
Piece by piece analysis:
Part 1:
They feel that their demons taking over but nobody is able to see that they are. "They" representing those demons
Chorus:
This could either be someone who is saving this person or a person that the singer is hoping for that can help them escape from their demons
Part 2:
Back to talking about the demons again. When they mention that they can see "you" out there, they could be talking about the same person in the chorus, although this may show that the person is hypothetical because of how they say "I can feel you out there" or "I know you're watching". This could also be someone in their life that isn't helping them but knows of his demons. It's kinda complex and ambiguous, but I think it could be either.
