Wish I could put properly into words how much this song means to me (especially the remake on the Japanese edition of the Isolate album). I'll try it anyway. I believe this song is about a man who is presently very dissatisfied with his own life. He wasn't true to himself during his life and now has to face the consequences of it.

I'm staring out the window It's raining outside The fall has caught up with me I feel that I can't breathe, wished I couldn't see I lived my life through someone else Long ago I made a choice that I thought would make a difference and lead me back on track

In the first stanza, the speaker finds himself beholding the autumnal rain from his window. In this case, fall is the season that precedes winter, which would be the end of life (i.e. death) or the end of a period (whatever the following results might be). Therefore, he sees himself close to the end and then goes over all his life, trying to make sense out of it. The ending suffocates his thoughts and he wished he didn't have to face time catching up with him. He regrets the choices he made in the past thinking they would do him well and "lead him back on track".

Then, we have the chorus. The speaker feels abandoned by the angels who should be looking out for him in the time he needs the most and doesn't have anyone to "hold close to his heart".

I'm standing at a crossroad I don't know what to do I will not live to see another day The time has come to face the end This is a necessary thing 'Cause I will hold your hand and say, "Will you forgive me?" Long ago I made a choice that I thought would make a difference and lead me back on track

In the second stanza, the speaker has a hard choice to make and doesn't seem to be able to decide. It could be either ending everything (which could easily mean committing suicide, but also checking out of a sane life) or embracing the choice of not living through someone else anymore (even for the last moments of his life) and accepting its scars and consequences, whatever they might be.

Either way, life continues to pass him by and he's just another leaf falling onto the ground. And winter is coming.
