1 he went overdose with his girlfriend. When this happens your muscles contracts and you feel like you can't breath. You actually can't breathe for a while.
In the end he won the fight, but she died
2 He went back to reality. No more drugs. He's no longer a shark and has to live out of his matrix. He feels like a fish out of water. And it's fucking hard to breathe according to that, plus the weigh of his guilt.
