I think this song has nothing to do with Metallica, it sounds nothing like it does.
What I think it means is that Dave is a "victory."
"Had needles in my veins" means the problems with the alcohol. "A knife right through my heart" is everybody who has broken his heart. "I am a victory" means he is victorious to be alive, and lucky not to be dead.
