This is mt favourite of his songs and to be honest I don't actually know what the song means so I'll give you my interpretation of it.

Overall to me the song is sang to someone dying, or the relationship dying.

Here, "them" is death, and they want to be close to them but their arms are shaking beside they're so emotional.

If you must weep, Do it right here in my bed as I sleep

People usually cry while they are dying, and he want to be with them, but he doesn't want to see them cry, so he's asking they do it while he sleeps

If you must mourn, my love Mourn with the moon and the stars up above If you must mourn, Don't do it alone

Once again he doesn't want them to be alone, as they mourn their own death.

He's saying if they're going to die on him they should do it quickly, as in its probably more painful to watch them die

These are her last words so he's asking her to choose them carefully

He's saying she she should put her all, and instead of arguing with him, she should work on making herself better

If she's going to do something that will cause strain she should do something that counts, she should leaver her mark, so he and others can remember her

He just wants her to survive, because it's painful watching her die.
