First off, love the song to death (get it?) and can't stop listening to it. The video is kick ass, too. Anyway, onto the interpretation!
The song is obviously about some guy who feels extremely guilty about possibly killing someone - maybe related to the Holocaust, considering Josef Mengele was considered the "angel of death"? - and is struggling to return to his daily life with the heavy guilt following him. It gets to the point that he's come to the realization that no matter what he does, the guilt will never fade and is willing to succumb to it by calling upon the "bloody angel" to put him out of his misery. The angel being Johannes in this case. Mmf! So good.

Having not seen the video, listening to the song always made me think it was about a guy who had found a murdered woman as a child. The "bloody angel" being whoever killed her; his screams taking him back to childhood, and having seen enough to feed psychosis for a lifetime. The bloody angel gave him his psychosis type thing.
