It seems the song is talking about a struggle with either a faith or a relationship during a dark time in the person’s life. The song begins with:

Knocked off my feet The earth moved beneath The edge of a dream and a nightmare

This illustrates how he fell down a dark path and has a hard time believing he got himself into it. He describes it as “the edge of a dream and a nightmare”. In the next verse, he admits that he chose to go down this dark path:

I opened the door

He also admits that this has destroyed him:

I slipped through the cracks into nowhere.

On the chorus, he is crying out to either God or a loved one, asking where they were when he went through that dark time.

Things get more intense later in the song where he makes a reference to self-harm and describes his smile as an illusion to what he truly feels:

This time, the chorus harbors some resentment when he accuses God or that loved one of leaving him when he needed them most:

The rest of the song continues with him questioning where God or that loved one was when he went through that dark time. He also claims that the ‘pressure’ took him over. What I believe he means by ‘pressure’ is a battle with depression, anxiety, addiction, or etc.
