To me personally its about Suicide. Whether it be about drugs(overdose),or like me a bullet survivor. The reason I think it's about suicide mainly with a gun, is because the lyrics and even the song title, 'Bullet in my hand.' The darkness is depression that ultimately drove him to suicide. And something saved him. He awoke in the hospital seeing the 'light', basically what drives him to do good now.
'A cold stare, and the wound's still there.' Is basically him being dead. 'The long black car', is a hearse waiting on him to die.
Whoever or whatever saved him gave him a second chance at life, and he's lucky to be alive.
