I take it as being that they are acting childish in a relationship with their partner. Perhaps having "tantrums" over trivial matters that are threatening their relationship.
I would say the line about the "baby coming out" could be literal or metaphorical. That is, literally there is a baby on the way and that they feel they'll grow up (behave more grown up) with the added responsibility of a child. Or metaphorically that they are they will grow up (the baby will come out of their behaviour).
They at least know that they are acting childish, but sometimes it is hard to address the matter in the heat of the moment ...
