I think this song isn't about a person named Isabelle. In fact, I think the song refers to drugs or pills. "You taste just like you always do" means the taste of the pill, it always tastes the same and has done ever since the beginning. "If there's still evidence of us" means that the person is trying to give up the pill, but is addicted to it and just searching for a way to say 'no' to it. Anyways, that's what I think the song means, feels free to comment.

My Interpretation

@joshalow on the band's website, it says the song is about a real person who had depression. The line "I won't be here in a year" is a real quote from her assumed to mean she was going to kill herself by that time next year. Happy ending though, as it also says she pulled through and is still around :)
