I think it's about drugs, as in a guy gave his whole life away to drugs, and became totally addicted, while others around him, such as friends, realize he has lost everything. The line "you broke your back, she broke your heart" I personally interpret as he has given everything for the drug, but the drug just completely destroyed him.
