there are definitely a few ways to interpret this song. One of the least likely ones is that it's about cancer. The talk of doctors, hair falling out, etc...but the most obvious one is depression. The person in question really needs and has probably become addicted to pain or depression medications, and now needs more and more expensive doctors to help her. A lot of people with depression don't want to let people near them or let them love them for fear that they will get hurt. My guess is that the artist is on the receiving end of this, and is desperately trying to help his girlfriend. The snow piece is a bit tricky: 1. Snow and winter are usually associated with sadness and depression, and the black river below could mean where she was before the medications

  1. Snow melts and disappears very quickly, especially in warm hands. "Down to the black river below" probably means that the girl is so sad that one day she could just have disappeared in the blink of an eye. The use of snow and river is especially interesting because of the contrast: snow is tiny and individual, while a black river sounds huge and unrelenting.
