The chorus immediately made me think of "gaslighting" in an emotionally abusive/manipulative relationship. One person says something awful, and when the other mentions it, the abuser turns it back around on them. They were only joking! Gosh, you clearly have no sense of humor. Or else they never even said that to begin with ("What are you smoking?") and you're remembering it completely wrong.
That combined with the rest of the song reminds me of being in a relationship with someone who's mentally unstable, either due to drugs or illness. They ramble on about really bizarre theories, and when you question them about it, they jump into defense mode. Once again, you're the "crazy" one for taking them seriously and worrying about them. OBVIOUSLY they don't REALLY believe that. My best guess is that this song is about one of those conversations. Even the pacing of the song suggests that same kind of frenetic energy from someone who's on drugs or experiencing active psychosis or mania.
