The writer is talking about his sins, he says its hard to stop from dropping them on you ("you" meaning Jesus who took the weight of the world, the sins of every person)

This right here is by far the deepest verse in the song, in the bible we as people are reffered to as trees in multiple verses (ex. Psalms 1:3, Psalms 52:8 and many others) with that explained, "the heart" refers to Jesus heart. The leaves refer to sins. So what I think the writer means is that Jesus heart aches of all the sin we have committed and will commit.
Anyway, Jesus loves us all (regardless what we believe in) dont ever lose faith :D great song from an amazing band its 2014 and im still jamming to em peace you all much love and God bless :)
