I think it's about trying to get over some traumatic life event. The birthday in question marks two years since the event happened. "Happy Birthday" is said with a scornful tone, since the birthday marks the day he was forced to run away from his old life - "two years dead today" - and become the metaphorical "burning baby" that was created in its place.
No matter how much his new self is determined to run away from what happened, he is still haunted by it. And it makes him ANGRY! Anger at what happened is now what's driving him forward, so much so that he sees his new life not as living but as "burning higher and higher" into the future.
