When one quits a powerful substance, there is a liberating feeling. A feeling similar to that of having handcuffs taken off. It literally makes your guts ache and your stomach feel like knives are stabbing it. He's got "guilt, fear, regret,"etc., because of the things he did when he used the drugs, but now he is free to do as he pleases.
