To me this song has to do with Heroin addiction (Much like China white)
The first verse depicts how Heroin has you swinging by a noose, or how Heroin can slowly kill you. Followed by a depiction of a very sketchy to say the least hotel which is basically saying how most heroin addicts can find comfort in the weirdest of places just to shoot up.
Thus into the chorus part inject Black tar etc. Then the "You always knew just what to say" which is kinda the narrator saying Heroin always comforts them and always knew how to.
