To put it short, He's tired of her and is giving her one last chance before leaving.
Oh, I've packed my things a thousand times, oh-ho-oh
This means he's thought about leaving alot, packing his things to leave - Then staying.
And I've faked believe all her lies, for the last time
This part means he has been pretending to believe lies to make her happy, and no longer will he.
The other day I drew myself a map So I could figure out your head
This means she gives off mixed signals, reacts easily, and is hard to understand, He's trying to understand her but it's impossiable.
The canvass dripped with my sweat, blood and tears And she hated every thread
This part is saying even when he works hard and trys, crys, bleeds, and sweats she hates it. She hates it despite his effort.
The first verse tells about the problem.
Take it or leave it
Means he's giving her a chance. Take him or leave him.
I've purchases my ticket to newyork means he's already about to go. It's almost to late. Take or leave?
Hopefully this helps, PM me if you'd like a further explanation.
