I see this as how a person (a person that wants to be compassionate) sees someone less fortunate perhaps a homeless person. In the chorus the less fortunate person maybe in the conscience of the observer says that they're not Invisible. The observer speaks of them as "them" and in the end realizes "they" are us.

I think of Matthew 25:35: For I was hungry, and you gave Me something to eat; I was thirsty, and you gave Me something to drink; I was a stranger, and you invited Me in
