the song seems to be about drug addiction and depression but i think the;

means that the person or friend is stuck with the same problem and cant find the solution/answer so they turn to drugs and alcohol this cycle/circle just puts them in the same place and they cant break it, while the other friend is stuck watching them and cant do anything.
