The basic plot for people who have trouble parsing these things out:
two male friends get into a bad car accident somewhere in rural America. The car has rolled onto the train tracks but both men are too injured to get out of the car and do something about it. The passenger confesses to the driver that he slept with the driver's wife. He wants his conscience clear since it's likely they'll be struck by the train and killed. However, after his confession, the passenger spots a man walking on a hill above them. The passenger tells the driver to blow the horn so the walker will find them and get them out of the car before the imminent train appears. But the driver is so devastated by his friend's confession that he decides he would rather have the train kill them both. Cold!
