It's talking about the Dnepropetrovsk maniacs, who were responsible for a string of murders in June and July 2007 in Ukraine. The killers made video recordings of some of the murders, and one of the videos got leaked online. Two local nineteen year olds were arrested and charged with 21 counts of murder. They reference this in the song when they say "Innocence turned its head at 19 as the two screamed". When they repeatedly state "fame" in the song, I believe it's because the teenagers were doing it for some sick, twisted aim at gaining fame. "With every cut they lift you up" could be read into meaning that with each death, they were "lifted" higher into their "fame". "21 counts of murder and the public eye" references the number of deaths and addresses the public awareness, which leads me to believe that it is in fact the maniacs that they are talking about. Later in the song they also state "16th of July, Ukraine saw the horror that rests aside", making it even more specific.
