The line "Here we are, juggernaut" is taken directly from the novel, in the first chapter.(Chronologically, it takes place around the middle of the story).

So I think Juggernaut is kind of like a pet name used from a husband to a wife. Given that, I think each verse is supposed to be from a different part of the story, from the perspective of either Coheed or Dr. Hohenberger.

Claudio explains that he wrote this song partially about him and his wife, and that the phrase "Here we are, juggernaut" Is like saying "Here we are, together, this unstoppable force".
