It's been too many years since she has been in love. She knows him better than he knows himself. He could pick it up and start over with her, but it's irrelevant to him. His Medicine may be literal(drugs alcohol pills) But i think she means his demons- his habits. He strays from what he knows will cure him which is her love & chases after his demons.

That's how I thought of it. It's like she's watching his heart control him and break himself apart though he's such wonderful person to her.
