To me, this indicates that his mental defenses are broken and a calm has come over him, all the while barely keeping afloat during his 'swimming hour'.
The effects have taken their toll, and are 'all contributions to my attrition'. The last lines are powerful, being so full of emotion, and confirming that this struggle has left a long-lasting physical imprint on him.
