I feel sorry for the people who read the only other review here and took it as fact and even more sorry for the sadly misinformed person who wrote it.

This song isn't about opiates, that's only one interpretation of it and probably the furthest one from the actual meaning too.

The meaning of the song in its most general sense is about going through a change and then more specifically a change of identity.

At the beginning of the song, he talks about his internal or emotional change which is so powerful that it also affects him physically.

"I feel it right at the heart of me and it grips me physically"

The more he tries to hold back the internal change in though, the more it difficult it becomes.

"Dragging down so far below And it's a violent vertigo

Digging it in like a parasite Feeding off what I keep inside

Manifest when I'm throwing trust To the cold and the nebulous"

He feels the need to change and finally decides to act on it and allow that part of his identity to change.

"Something's changing Re-arranging me Just beginning my own surgery

I'll cut away everything And tear away everything"

His old identity was confining but he's becoming more defined in his identity now that he's finding himself and changing accordingly to the point where he can escape his old self.

He doesn't want his identity to be changed by circumstances or any other influences like it was in the past. Now, he only wants to be changed by his own will. He's not going to dwell on the times when he allowed himself to be influenced and he's going to move past his experiences towards a fresh start.

Moving past the experience Never dwell on the evidence"

He's finally breaking off the part his identity that he no longer wants and he's putting it in the past as an experience that he had to go through in order to become himself. He's also struggled with deciding whether or not he should allow the change to happen, almost as if he's fighting with himself but after his metamorphosis it will just have been a struggle that he can put in the past with his old identity.
