This is clearly (?) about someone with mental illness problems.

The 'aching brow' could just indicate a headache, but then we hear about
it taking an hour to 'take him down' (apparently being in agitated state).
Trying to force the 'arms'(not eyes) out sideways is hindered by a straightjacket
The narrator also doesn't recognize himself in the mirror (and maybe has vision
problems, posisbly doublevision, indicated by 'them')
The 'first impressions often lie' line could however mean that the narrator is falsely considered mad (and kept drugged/restraint). This could be supported by 'it don't help me' and 'I've lost control', though all this could also apply to really metally ill person.
Thus the sone leaves us wondering whether this is a cry for help by someone is mentally ill or by someone who is falsely (maliciously?) treated as such...
