Like most of Wainwright's work, this works on several layers. The parody aspect is there, for sure, but it's also worth noting that these aren't the thoughts of someone who is alright, ok and has no problems. Someone who's genuinely fine doesn't look at the contents of a medicine cabinet and think of the myriad ways he could use them to kill himself. I think this is very knowing of Wainwright - his songs typically come from pretty dark places, even the seemingly fun ones. I believe that, with these protestations, he's intentionally letting on that he is both obsessing over his ex, and borderline suicidal. It's another eloquent masterpiece from one of the best songwriters extant.
