I've always imagined this song was about a girl catching the night bus home from the hospital where somebody she loves lies in a coma or severely injured idk. And so she keeps telling her she's not alone despite the strangers all around her. And she's not phased by the darkness in her soul, because she got used to it, the trouble had been there since forever (idk maybe she had hard time growing up). She's talking to herself to console herself, so she doesn't feel lonely.
But it might as well be more general, about a girl who's alone and detatched from reality, living her daydream, surrounded by imaginary friends ('I'm not alone'), and the whole story is told from the perspective of someone (boy?girl?) who usually takes the same night bus and notices her at some point, sees all the trouble in her eyes.
