Especially the line "don't worry now, 'cause it's all under control" which seems as though it's mimicking what a doctor would say.
The part about poking a voodoo doll could also be used imagery of medical practitioners trying new procedures on patients without thorough testing.
The line "moping and hoping around the street again" might have to do with not being sure of your health, and "tripped in dirty water, tap" could be a metaphor for suddenly and accidentally getting a disease and being forced to see a doctor about it.
It could be about more than this, perhaps the parts about cuts up and down his legs is about self mutilation, as well.
