When first hearing the song I thought that "addiction"-stuff could be taken literally, as if the song is about someone who was addicted to something for a long time and somehow fought his way out, or is still fighting. It could be an actual addiction to drugs, or perhaps to a certain person or whatever.
The "if you chose life"-lines sound to me as if someone experienced a life-or-death-situation, perhaps has been suicidal, and now has to live with some kind of fear (caused by the addiction?) and has to deal with it.
