To me, this song is about how, when you find yourself addicted to cocaine, whether you smoke it, inject it, or snort it, it becomes your whole life. Everything you do revolves around cocaine. When he says "hey baby you better come here quick, this old cocaine gonna make me sick" he's talking about craving that high that comes with the drug, to the exclusion of everything else. Near the end he talks about how the doctor tells you that its gonna kill you but he doesnt tell you when, I believe he's referencing the fact that when you're in that vicious cocaine fueled lifestyle, you are very aware of the fact that its going to kill you, but you dont know when and it could happen each and every time you choose to indulge, whether it happens in the form of the police taking you to jail, another crazed addict taking your life, or the meaningless repetitive cycle in itself of living to get high and needing to get high to live.
