In other words, it sounds as if the narration is indicating that she will regret this decision and she will harbor guilt over it for the rest of her life. I think that the "death is fucking you insane" line is very graphic. The reason I say this is because I think that Araya is saying that in order to cause the "death" of the fetus, the woman essentially has to be violated in a way. With the vacuum/suction aspiration method of abortion, they have to go in through the woman's cervix. I think this is why Tom is using an f-bomb here instead of saying that it's "DRIVING you insane." Essentially, the woman is being violated in order to achieve the death of the fetus. That's my take on this song.
