Personally, I'll take a lot of aspects from many of the other comments:
I think rather that someone he knows or knew was being abused, as shown in the 3rd line of the verse. Perhaps a sibling, and he was abused by an adult the adult that abused this other sibling of his. Now, I don't know if he has siblings or not, but it's a possibility.
Abuse is definitely evident. The question is, who's doing the abusing.
He notes that "life" in its self is falling away from him, perhaps that he was dying on the inside as the "hollow" part suggests, because of such abuse.
