It sounds like this song is about two people who are in a relationship in which they pretend everything is great between them, when they really have problems they are ignoring. And he can see it all happening, but can't do anything about it.

"You’ve got your Hell’s teeth smiling at you."

This line explains that the other person knows there's a problem, but chooses to ignore it.

"It keeps your brain safe as it all eats at your face."

Ignoring the problems helps her feel better about the situaion, but it destroys who she is inside

He says they will ignore it together, and pretend nothing is wrong between them
