I think this is from the point of view of a guy in a relationship with a woman who is self-destructive. He cares about her enough to be affected by her self-destructive nature, but because of her issues (whatever they are - they don't seem to be mentioned in the song) he's not seeing a lot of emotional investment into the relationship from her side.
