I think he's singing about a woman who is ill and knows she will die soon, the lines "You said you were quitting after your next pack" indicate she probably has cancer (also not the first time Middle East has sung about cancer) ..the relationship seems quite pressed i think, he drinks a lot and could never really tell her how he felt whereas she seems to be able to tell him everything
