I think it's about fighting life difficulties. It's about someone going through a hard life situation, maybe was even being close to death... I don't agree the hero finally dies though. I think the screaming power of the chorus represents the great strength and will to survive. The ending of the song sounds quite positive, and it's what represents the success for me.
