i think this could be from the point of view of the person who discovered the victim of part i:

for the "O do you want my heart between your teeth..." stanza, imagine the saviour is telling this to the victim, who he's wrapped in a blanket or whatever like "is there anything i can get you?", desperate to help this poor person in her fragile state (floating leaves).

the second stanza is like his adoration of her beauty, and the apparent futility of trying to give her the love she deserves due to the damage she's taken in her past - like swooning forever over an unpluckable flower or an untetherable bird.

the ending part is perhaps him shouting in outrage concerning all the people who have abandoned her - the rapists, her children - which disturbs the neighbours or something and she dies perhaps in accordance with the two hearts no more thing i dunno. harrowing pair o songs
