I think this song is about being in love with somebody. Perhaps having been with them before and never have gotten over them, or perhaps admitted that you loved them and them maybe not loving you back, so you pretend like you're over them when you never really were.
A lot of it, I think, has to do with those moments when you're holding your tongue about loving someone and putting on a facade when really you're broken and hurt inside. Maybe subtly showing your pain and hoping that they notice, but they never do.
