That's what made me realize it was about The Joker and Batman, it reminds me of the scene in Dark Knight when Joker is hanging over the edge of the building and Batman could have killed him, but he didn't.. I imagine Joker saying that to him and that's why Batman didn't kill him.
Regardless of the lyrics and meanings, it is a fantastic song!
