I think it could be taken literally, as if Kate is talking to a friend hanging on to life in a coma. It could also be interpreted metaphorically though--like all she wants is to be accepted by the person and she cares about them terribly, but the person is dead and unresponsive to her. She recognizes that they don't care for her but it's really important to her that they do. It's a great song, anyway. I love An Horse.
